Commonwealth Home Support Packages (CHSP)

Funded as follow:-

Over 65 (50 years and over for Aboriginal and Torres Strait Islander people) – Dept. of Social Services and

Under 65 (under 50 years for Aboriginal and Torres Strait Islander people) – Dept. of Communities, Child Safety and Disability Services; Queensland Community Care Services (QCCS).

 

Flexible packages of care are available for:

  • Frail, older people and people with a disability, especially those at risk of nursing home or hostel care because they are no longer able to manage at home.
  • Carers of clients may also be eligible to use the service.
  • Referrals are accepted from Doctors, hospitals, community agencies, relatives, friends, neighbours, carers and self.

Volunteer Program

Funded through Disability Services Queensland, Commonwealth Home Support (CHSP) Program. Volunteers provide complimentary services to the clients of Flexi Care Inc.

Podiatry Service

 

Funded as follow:-

Over 65 (50 years and over for Aboriginal and Torres Strait Islander people) – Dept. of Social Services and

Under 65 (under 50 years for Aboriginal and Torres Strait Islander people) – Dept. of Communities, Child Safety and Disability Services; Queensland Community Care Services (QCCS).

 

This service provides a flexible and responsive approach to podiatry needs of people who are eligible to receive CHSP services and who live in the South-East HACC Network area. Services are provided at a number of FCI clinics which are located in the South-East HACC area.

Home Care level 2 Packages (HCL2)

Funded through Department of Human Services.

 

These packages are targeted to older people living at home who:

  • have been assessed as eligible to receive a HCL1&2 by My Aged Care RAS Regional Assessment Team;
  • have complex care needs;
  • have a need for ongoing monitoring and review of changing care needs; and
  • Are referred from “My Aged Care”.

Veterans’ Home Care Program

Funded through Commonwealth Government, Department of Veterans’ Affairs. This service provides packages of care to eligible clients referred from the Veteran’s Home Care referral agency.

Flexi Care Inc. has a number of community partners they work with to ensure older people and people with a disability within the community receive ongoing support to continue to live in their own homes.

Hospitals

  • Queen Elizabeth II Jubilee Hospital (QE2) – QEII Jubilee Hospital is a public hospital and part of the Metro South Hospital and Health Service.
  • Princess Alexandra Hospital (PAH) – The PAH is one of three tertiary level facilities in Queensland, providing care in all major adult specialties, with the exception of obstetrics. It is one of Australia’s leading academic and research health centres.
  • Sunnybank Private Hospital – Sunnybank Private Hospital is a vibrant modern hospital that contributes to the provision of a range of coordinated healthcare services designed to deliver and improve the health and well being of the community it serves.
  • Greenslopes Private Hospital – Greenslopes Private Hospital is one of Australia’s largest private teaching hospitals owned and operated by Ramsay Health Care offering a comprehensive range of inpatient and day services across all speciality areas
  • Mater Hospital Brisbane – Mater Hospital Brisbane is jointly funded by Queensland Health grants and revenue generated by Mater Private Hospitals. It was established in 1911 to provide for the healthcare needs of the Brisbane community.

 

Retirement Villages

Flexi Care Inc. has various partnership agreements and Brokerage Agreements in place to refer clients / consumers to a multitude of Retirement villages.

 

Community Groups

Flexi Care Inc. participates in the Older People Speaking Out (OPSO) community forums regularly.

Flexi Care Inc. is an active member of the Younger Onset Dementia State-wide Working Party and facilitates information stalls at various community events e.g. forums, expos etc.

Flexi Care Inc. is an active member of LASA and hosts / attends various network meetings.

 

Other Service Providers

Flexi Care Inc. has established and continues to maintain effective professional collaborative partnerships with a myriad of providers in the Brisbane South region. This assists Flexi Care Inc. to provide innovative, flexible and targeted services to clients in collaboration with its partners.

Flexi Care Inc., facilitate and host a quarterly provider network meeting and these meetings are represented by 30+ providers, community organisation representatives and advocacy services.

 

Assessment Centres

Flexi Care Inc. works closely with “My Aged Care” as well as the Regional Assessment Teams.

 

Church Groups

Flexi Care Inc. has established and continues to maintain effective professional collaborative relationships with a myriad of church groups in the Brisbane South region.   FCI provide talks on Podiatry and submit articles through newsletters.
